Part 7: Stress Fractures: Why Your Bone Is Barking and How to Heal It for Good
Welcome back to The Overuse Injury Playbook. I'm Dr. Keirstyn, a chiropractor and endurance coach. In this series, we are breaking down the most common injuries that sideline athletes. Today, we are diving into one of the most dreaded diagnoses for any endurance athlete: stress fractures.
It starts as a mild, localized ache in your shin or foot during a run. You assume it is just another tight muscle or a case of shin splints, so you try to run through it. But unlike muscle soreness that warms up as you move, this pain gets sharper with every mile. Soon, it hurts just to walk around your kitchen, and pressing on one specific spot on your bone makes you jump.
This is the classic warning sign of a stress fracture. When you ignore the early warning signs of bone stress, you turn a manageable issue into a season ending injury.
What a Stress Fracture Actually Is
Our bones are dynamic, living tissues that constantly remodel themselves in response to stress. When you run, the impact causes microscopic damage to the bone. This is normal. Your body responds by breaking down the damaged bone and building stronger bone in its place.
A stress fracture happens when the rate of bone breakdown outpaces the rate of bone rebuilding. If you do not give your body enough recovery time, those microdamages accumulate. Eventually, they form a tiny, hairline crack in the bone. In runners, these most commonly occur in the tibia (shinbone), metatarsals (foot bones), or fibula.
Why Your Bones Are Breaking Down
Bones do not just crack for no reason. A stress fracture is a clear sign that your training load has exceeded your skeletal capacity. Here are the primary culprits:
The Too Much, Too Soon Trap: Drastically increasing your mileage, adding intense speed work, or jumping into hill training too quickly does not give your bones enough time to adapt to the new forces.
Poor Shock Absorption: Your muscles are your primary shock absorbers. If your calves, quads, and glutes are weak or fatigued, they cannot absorb the impact of running. That force travels directly into your bones.
Nutritional Deficiencies: Bone health requires adequate energy intake, calcium, and vitamin D. If you are under-fueling your training, your body will pull nutrients from your bones, making them fragile.
How to Actually Heal a Stress Fracture
Healing a stress fracture requires patience and a systematic approach. You cannot run through this, but you can maintain your fitness while you heal.
Remove the Impact: This is non-negotiable. You must stop running and any other high-impact activities immediately. Swap running for swimming or deep-water running to keep your cardiovascular system active without loading the bone.
Build Muscle Support: Once you are pain-free with daily walking, start building the muscular support system around your bones. Use the Glute Bridge to build hip strength and the Hip Hike to stabilize your pelvis.
Strengthen Your Lower Legs: Weak calves contribute to shin and foot stress fractures. Incorporate calf raises and single-leg stability work like the Single Leg Deadlift to build resilience in the lower chain.
Address Your Fueling: Ensure you are eating enough to support your training volume. Focus on nutrient-dense foods and check your vitamin D levels.
Manual Therapy and Assessment: In the clinic, we analyze your running gait to find the mechanical flaws that caused the overload. I use soft tissue therapy and joint mobilizations on the surrounding joints to ensure your body distributes impact forces evenly when you return to running.

A stress fracture is a tough diagnosis, but it is also an opportunity to rebuild a stronger, more resilient foundation. By addressing your strength, mechanics, and recovery, you can come back as a more durable athlete.
